After having her father and perverse stepmother Julia (Laurence Olivier Theatre Award winner CLARE HIGGINS), killed by the cenobites, Kirsty Cotton is sent to a psychiatric hospital. Even though trying to convince the authorities of what really happened with her family, nobody believes in her version of the history and they decide to place her in the institute, so that she may be cared. The hospital is commanded by a brilliant and cold psychiatrist, Dr. Channard (KENNETH CRANHAM), who has been secretly seraching for the key to the dimension of pain and erotism for a long time. The only person who believes in Kirsty is a young and weak-willed doctor, Kyle MacRae (WILLIAM HOPE). Following Kirsty's version of the story, Channard put his hands on the bloodstained mattress where her stepmother Julia died. Channard decides to resurrect her, killing his patients and offering them as sacrifice for Julia. She returns without skin, and she decides to help Channard to bring the forces of the cenobites. Kyle and Kirsty start to investigate and go looking for clues inside Channard's place. Kyle goes upstairs, while Kirsty stays behind. Kyle is locked by Julia inside a room, mercilessly humiliated and then get his blood drained by Julia. Although in great pain and his moanings and struggle, Kyle is dominated and taken care of easely by Julia. Channard uses a mute and brilliant patient to solve the Box, unleashing the cenobites and their dimension of sex, pain and humiliation.
